About the opportunityWe are currently seeking a committed Clinical Care Coordinator to join our Clinical Leadership team at our Moran Kellyville Home. Reporting to the Care Manager you will be responsible for the overall clinical assessment program, and supporting the RNs as required to ensure resident assessments are completed and inform the development of care plans tailored to resident’s individual assessed care needs. As a member of the Clinical Leadership team, participates in clinical governance activities, including planning and implementing clinical priorities.You will be supported and mentored to ensure you thrive in your role with us. Key Responsibilities:Working closely with the Care Manager (CM) you will establish a Care Plan review program ensuring care plans and assessments remain current and relevant, reviews are initiated when required and residents assessed care needs are well documented.Participating in the development of training and education plans to ensure employees are provided with the required support and education to enable them to competently complete assessments, care plans and documentation based on accurate and comprehensive assessment, consultations, observation, and collaboration.Responsible for the completion of regular analysis of the use of psychotropic, antipsychotic, polypharmacy and antimicrobial stewardship and reporting and consent obligations.Assessment and care planning for new admissions and existing residents, in consultation with internal and external stakeholders, Care Team, health professionals and Admissions Coordinator to ensure a personalised care plan is developed.Monitor the delivery of care is consistent with the care plan and goals including dignity of risk choices and take action as required if gaps or inconsistencies are identifiedReview of care incidents and care change patterns for residents and collaborate with Clinical Care Team to determine if further assessment and care planning modifications are needed, initiate care plan reviews and re-assessmentsmaintain accurate client records, case notes and other data as directed and required.
