To find out more about us visit www.morangroup.com.au.  Our values are Respect, Kindness, Dignity and Compassion.  About the opportunityWe are currently seeking a committed Clinical Care Coordinator to join our Clinical Leadership team at our Moran Kellyville Home. Reporting to the Care Manager you will be responsible for the overall clinical assessment program, and supporting the RNs as required to ensure resident assessments are completed and inform the development of care plans tailored to resident’s individual assessed care needs. As a member of the Clinical Leadership team, participates in clinical governance activities, including planning and implementing clinical priorities.You will be supported and mentored to ensure you thrive in your role with us. You will work with a great team of dedicated people and your contribution will make a difference to our residents’ enjoyment of their life every day. Education in the new Australian National Aged Care Classification (AN-ACC) classification system for the successful candidate will be provided.If you are resilient and resourceful individual with passion to improve our residents’ daily life, this role is for you! Key Responsibilities:Working closely with the Care Manager (CM) you will establish a Care Plan review program ensuring care plans and assessments remain current and relevant, reviews are initiated when required and residents assessed care needs are well documented.Participating in the development of training and education plans to ensure employees are provided with the required support and education to enable them to competently complete assessments, care plans and documentation based on accurate and comprehensive assessment, consultations, observation, and collaboration.Responsible for the completion of regular analysis of the use of psychotropic, antipsychotic, polypharmacy and antimicrobial stewardship and reporting and consent obligations.Assessment and care planning for new admissions and existing residents, in consultation with internal and external stakeholders, Care Team, health professionals and Admissions Coordinator to ensure a personalised care plan is developed.Monitor the delivery of care is consistent with the care plan and goals including dignity of risk choices and take action as required if gaps or inconsistencies are identifiedReview of care incidents and care change patterns for residents and collaborate with Clinical Care Team to determine if further assessment and care planning modifications are needed, initiate care plan reviews and re-assessmentsmaintain accurate client records, case notes and other data as directed and required. Skills/Requirements:Registered Nurse with at least 2 -3 years clinical experience; strong leadership skills and knowledge of the Quality Standards for Aged CareA high level of knowledge and understanding of the Care Planning, Assessment and Documentation in Residential Aged CareExcellent and demonstrated verbal, written and interpersonal communication skills.Knowledge and ability to use computers and technology efficiently.Current Police Check and appropriate Working RightsUp to date vaccinations (Influenza, Covid-19)Experience in a similar position in Aged Care - desirableManagement qualifications – highly regardedAs a MHCG employee, benefits you'll be able to take advantage of: Australian National Aged Care Classification (AN-ACC) trainingEmployee recognition and benefitsMentoring by Senior LeadersEmployee Assistance ProgramEmployee financial well-being ProgramDiscounted Childcare rate for Moran Employees at Discover My World Childcare.Training and development OpportunitiesPaid Parental LeaveIf you are searching for a great career opportunity, dedicated to making a difference, please apply today, we would love to hear from you!At Moran Aged Care, we are committed to reflecting the diverse community we serve and creating an inclusive and respectful workplace for all employees.
